Assisted Living Program (Grace Place)

About the Program

Established in 1999, Grace Place is an assisted living program for Korean elders who experience language and cultural barriers. The staff members of Grace Place are attentive, caring, bilingual and sensitive to cultural issues for helping the elders to live more independently for 24 hours, 365 days.

Program Information

The Assisted Living Program allows elders to continue to live in the community by providing the following services. Residents of Grace Place receive weekly nurse visits, daily medication reminders, two Korean meals a day, and weekly housekeeping and laundry services. Grace Place also provides recreational and social activities such as bingo, movie nights, field trips, games, and crafts.

Open 24 hours a day, seven days a week, Grace Place offers elderly Koreans the opportunity to live independently.

 

 Qualifications

  •  Minneapolis Public Housing Resident (Or need to apply) 

  •  Qualifications: 65 years older or People with Disabilities 

  •  Citizenship or Permanent Resident Holder 

  •  Medical Assistance beneficiary / Private pay (if unobtainable)

 

Services Provided

  •  2 Korean meals a day / Breakfast / Snack

  •  Weekly Nurse Practitioner visit (Health Partners Health Plan)

  •  Grace Place Nurse Benefit  

  •  Medication set-up & administration / Reminders

  • Scheduling medical appointments and transportation

  •  Weekly housekeeping & laundry 

  •  24-hour emergency on site responder 

  •  Activities / Exercises 

  •  Lifeline service
